Medical Card ProcessGetting a medical card in Oklahoma takes two things. The first is a physician who will complete a medical form supplied by OMMA. There are two types of forms, one for adults and one for under 18 years of age. The form must be filled out correctly or you will be rejected. Minors will need 2 different physicians to fill out and sign a form. You have up to 30 days to obtain both signatures once the first form is signed. The caregiver part must also be signed by both doctors.
The second thing you need is to complete the application process online. This includes uploading information to the state of Oklahoma as well as your medical form and pictures of you and your identification. This is very important to do correctly or you will be rejected by the state. OMMA does offer videos and instructions online and for those who are uploading, it is full of great information. OMMA will take a passport or tribal ID for identification however prefer a drivers license or state ID. We have had some difficulty with Cherokee tribal cards, I don't know why. Self portraits with a white background are also required with no jewelry and no smiling. Both iPhone and Android have passport apps that can help in obtaining the proper picture. Make sure to upload Soonercare or Medicare cards since this drops the state fee to $22.50 compared to $104.50. You can log in to Soonercare and take a screenshot of your account.
